LPA observed a slip resistant mat was missing from the resident bathroom which could possibly pose a potential health, safety or personal rights risk to persons in care.

LPA observed cleaning solutions were unlocked underneath the kitchen sink and in the garage. This poses an immediate health, safety or personal rights risk to persons in care.
